A blank "notice of intent to vacate" letter could be included along with the lease in your tenant welcome kit. Standardize your move out so that all the information you need  is always available.

Lead Warning Statement
Lead from paint, paint chips, and dust can pose health hazards especially harmful to young children and pregnant women. Federal law requires you give all tenants in pre-1978 buildings a pamphlet highlighting lead paint hazards. Use this sample to document that you have complied with the law.
